What is the PD-1/PD-L1 pathway?
PD-1 (programmed cell death-1) receptor (CD279)는 활성화된 T cell의 표면에서 발현되며, ligand인 PD-L1 (B7-H1; CD274) 과 PD-L2 (B7-DC; CD273)는 일반적으로 dendritic cell 혹은 macrophage 표면에서 발현됩니다.
PD-1 과 PD-L1/PD-L2는 immune checkpoint 단백질군에 속하며 T cell 반응을 멈추거나 제한하는 co-inhibitory factor 역할을 합니다.
PD-1/PD-L1 의 상호작용은 chronic autoimmune inflammation을 최소화 하기 위해서 적절한 때에만 면역 시스템이 활성화 되도록 합니다.

정상적인 조건에서 면역계는 항암 면역 반응과 암세포 사멸로 이어지는 일련의 단계를 수행하며, 이를 cancer immunity cycle 로 알려져 있습니다.
1. Tumor cells은 돌연변이 항원을 생산하고, 이것은 dendritic cell에 포획됩니다.
2. Dendritic cell은 종양 항원에 대하여 T cell을 준비 시키고, cytotoxic T cell의 활성을 자극합니다.
3. 활성화된 T cell은 종양을 찾아서 침투합니다.
4. 활성화된 T cell이 cancer cell을 인식하고 binding 합니다.
5. Binding 된 effector T cell이 세포독성을 방출하여, 표적이 되는 cancer cell의 apoptosis를 유도합니다.
The PD-1/PD-L1 pathway represents an adaptive immune resistance mechanism that is exerted by tumor cells in response to endogenous immune anti-tumor activity. PD-L1 is commonly over expressed on tumor cells or on non-transformed cells in the tumor microenvironment2. PD-L1 expressed on the tumor cells binds to PD-1 receptors on the activated T cells, which leads to the inhibition of the cytotoxic T cells. These deactivated T cells remain inhibited in the tumor microenvironment.
